I am registering my child at New Horizons Montessori for Summer Camp.
Please list below, in preferential order, anyone other than yourself who has authorization to pick up your child. These individuals will be contacted in an urgent situation, including a medical emergency, in the event that parents cannot be reached. Please choose local residents.
**Please make sure to provide a copy of your child’s Universal Health Form and Immunization Records**

Week #4: July 26- July 30
Week #5: August 2- August 6
Week #6: August 9 - August 13
Week #7: August 16 - August 20

To comply with the Summer Camp policies, I understand that I will limit Attendance Change Requests pertaining to my child’s Summer Camp schedule to two changes (which depends on staffing arrangements and space availability in the classroom).
I hereby grant permission for my child to use all of the play equipment and participate in all physical activities conducted at the school. In the event of an extreme medical emergency, as deemed by the director or acting director, paramedics or medical personnel will be notified IMMEDIATELY to initiate medical attention for the child. All efforts will be made to notify the parents or guardian or persons named above for emergency contact, immediately. Due to insurance regulations, injured or ill children must be transported to a hospital, when necessary, by paramedics or ambulance. The child cannot be transported by school transport or school personnel. Upon immediate need for medical attention for your child the undersigned hereby gives consent to X-ray examinations, anesthetics, medical or surgical diagnosis or treatment, and hospital care to be rendered to said minor upon the advice of a physician and/ or surgeon licensed under the provisions of the Medical Practice Act. For this purpose I / We give permission to release my child’s necessary medical information to the facilities and also assume all medical care charges.
I/we understand that our child will be participating in water play that may include using the wading pool and/or sprinkler. Through this application, I/we allow my child’s participation in these activities.
I/we understand that the State requires the school to define an expulsion policy, which is stated below.
It is the camp policy that a child may be expelled from the camp immediately, if
- The child’s behavior in the classroom is disruptive for the class or is detrimental to the well-being of other children or staff or the behavior is dangerous.
- The child/parents do not comply with the camp policy and/or administrative directives.
- The parents/guardians’ behavior towards the camp administration/staff is aggressive/abusive.
However, if in view of the camp administration the circumstances do not warrant immediate removal of child from the camp, a time limit may be given for parents to find alternative arrangements for the child being expelled.
I understand that if my child will not attend the camp for the Full Summer months, the camp dues (calculated on the weekly fee basis) are payable in accordance to the procedure explained in the Summer Camp Enrollment brochure/email, and that the fee is non-refundable. However, it may be adjusted towards any other week/days of the summer program or Regular school year tuition, depending on availability.
I also understand that there will be no compensatory days for absence from the camp regardless of the reason.
I allow/do not allow my child’s image to be displayed on social media and/or school communications (like newsletter and School Facebook page) for informational/educational purposes.
If parents are separated or divorced, both parents signatures are required. In such a case when you press “SEND” both parents will receive an email with this application as an attachment. The attachment (application) should be printed and signed by both parents and then submitted to the school.

By typing my signature and sending this form using the internet medium I acknowledge that I have read and understood all the information provided; I agree and understand that my electronic signature is equivalent to my manual written signature.
I understand that our commitment to enroll our child for the Full Summer Camp months is confirmed and the camp fee is non-refundable. However, it may be adjusted towards any other week/days of the Summer Program or Regular School tuition (session starts in September) based on availability and discretion of school management. I also understand that there will be no compensatory days for absence regardless of the reason for absence. Also, since I am paying the July/August tuition as regular monthly fee, I will pay the additional monthly charge of $30 per special event conducted by outside vendors during summer.
I agree to follow the school/camp policies as described in the Parent Policy Handbook and Summer Camp Confirmation Letter
